What New York Women Want: Real results

The ultimate desires of the cosmetic or plastic surgery patient has never changed.  We all want to look younger, more well-rested; like the same person we are-but better.  The pendulum of cosmetic surgery however continues to swing back and forth every few years.

 

10 years ago, the most critical elements of the youthful face was being wrinkle-free, having full cheekbones, and good skin texture.  The relative importance of each component was in that order.  Today we find ourselves with the same concerns but in the opposite prioritization.  Today’s woman (or man for that matter) in New York City wants flawless skin, strong cheekbones, and wrinkles (although the fewer the better) are less bothersome.

Celebrities like Kim Cattrall and George Clooney has shown us you can rock out wrinkles with style and sex appeal.  The desire for improved skin still remains of paramount importance.

 

New York City women have no time to recover and want results that work.  This need has given birth to fractional laser technology.  The difference between a fractionated laser and an ablative laser is confusing to the average consumer, but is actually quite simple.  A fractionated laser delivers energy to the deep tissues in columns, leaving islands of unharmed skin to allow for a quicker recovery.  An ablative laser removes the unwanted tissue in a straight beam, leaving no such island behind.

 

Fractionated lasers come in many forms and fashions from different companies.  The selling point on these lasers is a quicker recovery time- often 3-5 days.  With these lasers, the results are not as dramatic than traditional ‘ablative’ lasers.  These fractionated lasers usually require 3 or more treatments to see appreciable tangible results.

 

Ablative lasers are the ‘gold standard’ of results in which others lasers compare themselves to.  With these lasers the results are often dramatically impressive.  Unfortunately, prolonged redness and discomfort will occur and can persist for months after treatment.

 

Dr. Scott Blyer, of CAMEO Surgery, has developed a technique to get the results of an ablative laser with the cumulative downtime of 2 or 3 fractionated laser treatments.  By a creative process using elements of your own blood applied topically, Dr. Blyer has made this possible.  Redness that used to last 2-3 months is essentially gone in 10 days.  Pain that was at one time rated as a 9 out of 10, is now described by his patients as 0 out of 10.  Patients after this procedure rarely take more than simply a Tylenol.

 

The questions Dr Blyer poses to his patients are these: “Would you rather 4 treatments with a laser that has lesser results or one treatment and be done?”  The answer is usually one.  His next question is, “Would you like a treatment with 10 days of downtime or over 16 days?”

 

Now it is time for artithmetic.  If you have 4 treatments with a laser, each with 4 days off, this adds up to 16 days of downtime.  This is more than the 10 days of downtime following an “aggressive procedure”.  Of course, less days is mores desirable. It makes sense.

 

Sun-damaged skin is repaired, wrinkles erased, and skin tightened without any surgery.  Dr. Blyer’s technique is the one procedure that can make a significant improvement in creating a more youthful appearance without surgery.  People often boast they look 15 years younger, as they are left with baby-smooth, shiny skin.

Botox, the scam, the myths, the dangers?

Botox is unsafe and can kill you

FALSE. Botox has been used therapeutically for 20 years. It was FDA approved in 1989 and we have over 2,300 peer reviewed medical articles written about it. The lethal dose of Botox is 3000 units. The average Botox cosmetic patient receives 100 units. This is a huge safety margin.

 

Botox is for the “rich and famous”, people like me don’t use it

FALSE. According to a recent survey of 1048 Botox Cosmetic patients:

  • working mothers between the ages of 40 and 55 are the most common users of Botox.
  • those with a household income of less than $100,000 (44%) were more likely to use Botox than those with annual incomes of more than $150,000 (35%).

 

If I stop using Botox, my face will look different than before

 

 

The effects of Botox can last up to 4-6 months. When the drug wears off, your face will return to its pretreatment condition. Many people choose to continue regular treatments (typically 2-3 times per year).

Botox can spread

PARTIALLY FALSE. When injecting, a small amount does diffuse and spread to the area around the injection area. This is more true with Dsyport. Much of the spreading of Botox has to do with the dilution the doctor prepares and his/her technique. This chemical should not spread further than an inch away from the injection site.

Does it spread throughout the body?

No. Patients often don’t understand this-sadly, either do many doctors. The way Botox works (simplified version) is as follows.

  • Normally a muscle contracts because it receives a chemical signal to do so. This chemical is released from a site “factory a” in a little bubble, and causes contraction at the site of muscle contraction, site “b”.
  • Botox binds to a port at factory “a” and is internalized. Once internalized, it binds to the little bubbles of chemicals inside the factory walls and does not allow them to be released to get to point “b”.
  • When Botox “clinically” wears off, it doesn’t spread at all. It is still doing its job inside the factory. Your body reacts by producing new factories in a close by area to send to point “b”. This chemical can then finally be released and the muscle contracts.

My doctor only charges this small amount for Botox for my whole face

I am sure your doctor is the nicest, most generous guy on the planet, but I doubt he is doing a procedure like Botox and losing a few hundred dollars to make you look good. People shop around for prices and come in with coupons and honestly it is mind-boggling. Here is how Botox works…

what your doctor doesn’t want you to know…

Botox comes refrigerated to the doctor’s office. (if it’s not cold-it’s not good, like the blue mountains on Coors)

It comes in a fine powder and is mixed by the doctor. (the only way doctors can turn a profit on some of the prices that are advertised, is diluting the heck out of the bottle)

A full treatment for a patient is probably 1/2 a bottle. (a doctor can improperly dilute it enough to treat 10 patients inadequately-I know many who do)

Botox should be discarded after 4 hours as per company recommendations (old Botox although I do not believe is dangerous, I do not believe works well. I know for a fact there are offices that save Botox for up to a year. This is not something we do, I assure you. It sometimes results in a loss of income if someone cannot be scheduled to finish the bottle in time, but we would rather have a happy patient.)

Botox is made by Allergan, in the US. This is the only FDA approved Botox. Some offices purchase Botox from Canada or Europe. It is cheaper. Fake Botox does not have the hologram on the bottle. Otherwise the bottle looks the same. You have every right to look at the bottle. It is being injected in your face!

 

I get my Botox done at a spa with a nurse

As long as that person knows what they are doing, has a good understanding of anatomy, and know how to handle any complications they can create, I have no problem with this. At my office, I am the only one who injects Botox.

Botox will make me look frozen

In the hands of an experienced injector, Botox will look very natural or as unnatural as you like. I personally like the natural look, where one can still can make expressions but the wrinkles are less.

To the general public:

There is tons of bad information out there about Botox. Please know what you are getting into. Look for someone who has have good patient feedback and not the cheapest price in town.

To my novice injectors out there:

My philosophy on Botox is every patient is a walking advertisement. Do not cut corners to try to make an extra few bucks. Do the right thing, and your reputation will spread, and it will come back 10 fold.

Belotero Balance® the newest FDA approved wrinkle eraser- a game changer

The newest weapon on the war on wrinkles, Belotero Balance®, received FDA approval November, 2011.  Merz Aesthetics, plans to officially launch it in early 2013.  Limited supply of this product has been issued in the US thus far and the product has been receiving positive reviews.  Its use has been in existence since 2006 in Europe.

 

Dr. Scott Blyer, a NY based Board Certified Cosmetic Surgeon, has been amongst the few privied to the product thus far.  Dr. Blyer speaks with excitement when asked about Belotero Balance®.  “I have been waiting for the release of this product since I heard of its potential a few years ago.  It is filler quite different from any other filler on the market”, explains Dr. Blyer. “While I typically use Radiesse® for adding volume to the cheeks and nasolabial folds, as the fold come to the superficial surface around the mouth, I have been unable to offer patients an effective predictable option until now.  I would always tell my patients, fillers were not made to chase wrinkles, merely to add volume.  This filler was made to chase wrinkles.”

Belotero

 

It falls into the category of a hyaluronic acid filler like Juvederm®, Restylane®, or Perlane® but has properties which make it quite unique.  The chemical nature of this product is a monophasic hyaluronic acid dermal filler with patented cohesive polydensified matrix technology.  This technology is based on a dynamic double cross linking of the monophasic hyaluronic strands with the stabilizer BDDE (1-4 Butanediol Diglycidyl Ether).  These properties allow for the filler to be injected smoothly, and very superficially while minimizing the risk of lumpiness.

 

Dr. Blyer further shares his experience with the tear trough area, under the eyes.  “Restylane® and fat injections were my only options to this area owing to the thin nature of the skin under the eye and close proximity to the orbital vasculature.”  The Tyndall effect is a complication of injecting predecessor hyaluronic acid fillers, when a blue halo under the skin was seen.  This was as a result of the clear filler injected under the thin skin of the eye.  “The chemical properties of Belotero Balance® allow for a safe injection under the eyes without this potential complication.”

 

The syringes come in 1 cc and because the injections are performed so superficially, little is needed to note an effect.  One syringe injected superficially can yield a more notable result than 2 syringes of a deeper filler. While there are constantly European fillers moving into the US market, many are redundant in their limitations and usages.  Belotero Balance® however, is a filler that can be used in areas doctors have regarded as taboo in the past.  These are the superficial lines around the mouth, lips, and the eyes.  Below the eyes, in the tear trough, a new door has opened which should allow patients another viable, predictable option for wrinkle reduction.  The results are variable and last beyond 6 months. While this is not a wrinkle panacea it is approved only for the lower portion of the face, off label use has been promising.
